New Patients

Thank you for scheduling an appointment with us. We look forward to meeting you in person and helping you get out of pain and back into the things you love.

Here are just a few items that will save time and help make your first visit more efficient.

Download, print, and fill out our new patient intake form
, with patient information and background on the reason you’re coming to see us. The pain diary is not required but will be very helpful if you have time.

Please bring your insurance card to your first visit. After scheduling your initial appointment, we will contact your insurance company to confirm physical therapy benefits coverage. We strongly encourage you to do the same prior to your appointment, to confirm coverage levels and determine if you need a referral prior to starting physical therapy.

Please wear unrestrictive clothing and sneakers for your visits. If you have any questions, concerns, or feedback, feel free to contact us.

Beware of GPS: It works if you put in Severance Corners, Colchester, but it does not like 47 Commerce Street (people have been sent to Williston or S. Burlington). See our Directions page for detailed driving instructions and video.

If you have any questions prior to your appointment, you can call us at 802-876-1000 or e-mail info@inspirephysicaltherapy.com.

The physical therapists at Inspire Physical Therapy know how to help you to engage and develop certain muscles and relax and stretch others in order to heal from and manage a wide variety of injuries and problems. Over a decade and a half, they have helped me through a frozen shoulder, back and neck issues, sciatica, pulled sacroilaic joint, dancer's hip, plantar fasciitis, issues with ankle osteoarthritis, sprained thumb, golfer's elbow, and TMJ. They have taught me how to find ways to work with my body and whatever problem it is having to help it to stabilize and maximize its function. Their knowledge of the body's musculoskeletal system, astute trouble-shooting, flexibility to find exercises that work with your own body's quirks, and their optimism and encouragement are truly inspiring.
